New Patient Forms
Please have your new patient registration forms filled out and submitted to our practice prior to your visit. If you have questions about submitting your forms please contact us at 518-489-4471 and speak to a Patient Coordinator.

Please call at least 24 hours prior to an appointment if you must cancel. We reserve the right to charge $125 for missed new patient appointments and $50 for a follow-up appointment.
If you arrive late for your appointment, you will be seen at the physician’s discretion.
Our staff and physicians work hard to ensure your appointment is on time. We understand your time is valuable too. Occasionally, delays cannot be avoided. Please be understanding as we attempt to give each patient the time and attention required to provide quality care.
If you have a prescription that needs to be refilled, please call your pharmacist. They have complete information about your prescription and are able to request refills electronically increasing speed and accuracy in refilling your medication.
We routinely contact patients only with abnormal results that need to be addressed prior to your next visit. If you have a portal account, you may access your lab results there.
Our providers are committed to making our office a resource for education. In this regard, you may encounter medical fellows (Board Certified Physicians in internal medicine training), residents and/or pharmacy and medical students. You may decline their presence if you so wish, but we appreciate the opportunity you provide them by allowing them to participate in or observe your care.
Because it is insecure and unreliable, e-mail should never be used to communicate with your provider. If you have access to the patient portal, your provider may choose to send you information through secure messaging available there.
Office hours are by appointment only and vary with each provider. We may be reached Monday-Friday 8:00am to 4:30 pm for appointment scheduling and questions.
Call 911 for life threatening emergencies. Do not call the office or leave a message on the portal.
In case of an urgent non-life threatening medical issue, a physician is available after hours for our patients and may be reached at our main phone number 518-489-4471.
For Albany and Schenectady Patients: The Center for Rheumatology does not require new patients to have a referral from their primary care physician. In the case of the patients’ insurance carrier requiring it then it is the patient’s responsibility to obtain the referral. If you or your patient is interested in a consultation with one of our professionals, please call 518-489-4471 and speak to a New Patient Coordinator. For Saratoga Patients: The Center for Rheumatology requires new patients to have a referral from their primary care physician to be seen at this office. Saratoga office staff will request patient records and demographics.
When all required records are received, staff will make sure that we;
- Participate with your insurance
- Have all the records needed
- Once everything is in order, a patient will be registered in our practice system and will receive a phone call from staff to schedule their appointment. A welcome letter is then sent out to the patient and includes new patient visit information.
