New Patients

Referral Policy

The Center for Rheumatology requires all new patients to have a referral from their primary care physician or another physician's office.

If the patients' insurance carrier requires a referral, it is then the patient’s responsibility to obtain this prior to the visit.

New Patient Fax Line

Referring providers can fax a completed Medical History Form to 518-533-1372.
